The successful candidate will work closely with multidisciplinary healthcare teams to assess psychosocial needs, provide counseling and support, and assist patients and families in navigating complex healthcare situations. Key Responsibilities • Conduct comprehensive psychosocial assessments of patients and families. • Identify social, emotional, financial and environmental factors affecting patient care. • Develop and implement appropriate psychosocial intervention plans. • Provide individual and family counseling and emotional support. • Support patients and families dealing with serious or complex medical conditions. • Assist families with coping, adjustment and decision-making. • Provide appropriate referrals to community, social and healthcare resources. • Work closely with physicians, nurses and other members of the multidisciplinary team. • Participate in multidisciplinary case discussions and care planning. • Maintain accurate, confidential and timely social-work documentation. • Advocate for patients' needs and support access to appropriate services. • Assist with discharge planning and continuity of care. • Support crisis intervention when required. • Participate in education, quality improvement, research and clinical governance activities. Requirements: • Bachelor's or Master's degree in Social Work, Clinical Social Work or a related recognized discipline. • Professional registration/licensing where applicable. • Significant clinical social work experience in a hospital or healthcare setting. • Senior-level experience providing psychosocial assessment and intervention. • Strong counseling and communication skills. • Experience working with multidisciplinary healthcare teams. • Ability to manage confidential and sensitive patient information professionally. • Candidates must meet the applicable DHA professional registration requirements, where applicable to the role.
