The successful candidate will contribute to the safe and effective use of medications, provide pharmaceutical support to patients and healthcare professionals, and participate in medication-safety and quality-improvement initiatives. Key Responsibilities • Provide professional pharmaceutical services in accordance with established standards. • Review and validate medication orders and prescriptions. • Assess medication appropriateness, dosing and potential interactions. • Provide medication counseling and education to patients. • Collaborate with physicians, nurses and other healthcare professionals regarding medication management. • Monitor medication safety and identify potential adverse drug reactions. • Support medication reconciliation and safe transitions of care. • Maintain accurate pharmacy and medication-related documentation. • Ensure appropriate storage, handling and dispensing of medications. • Participate in medication-use evaluations and quality improvement initiatives. • Support the development and implementation of pharmacy policies and procedures. • Assist with training and supervision of junior pharmacy staff. • Participate in clinical audits and medication-safety programs. • Maintain compliance with applicable regulatory and professional standards.
